WHAT TO EXPECT
In order to ensure that your initial visit to our office is as straightforward as possible there are a few things that you should know.

First of all, this visit will take about an hour. This ensures that we have enough time to do a thorough health history and exam in order to diagnose your problem.
​
There is some paperwork that needs to be filled out for this visit. Please ensure that you arrive a few minutes early for your appointment to fill this out. Alternatively, you can find our forms in the Exercises and Patient Forms section of our website. Please feel free to print those forms and bring them with you to your visit.

We may be able to bill directly to your extended benefits. Please provide your benefits information to our receptionist upon arrival at the clinic to ensure that they have time to process this information. Please note that although we may be able to bill directly to a company, all plans are different and we may not be able to do this for all plans

Please give us your insurance information when you arrive at the clinic so that we can verify whether we can bill directly for you or not.
